Beneficiary Nomination Form: This document serves as a crucial form for members of the pension fund to designate or update their beneficiaries who may receive benefits upon the member's death. This form is essential for ensuring that members clearly state their preferences for the distribution of their benefits among dependents and other nominated individuals. Significantly, this form should be completed at least annually or whenever there are changes in the member’s living circumstances, such as marriage, the birth of a child, or the death of a previously named beneficiary and the original should be kept on the member’s file with the Employer. It ensures that the fund has up-to-date information to assist the Trustees with the distribution of the benefits. Only upon death of a member should this form be submitted to the Fund/Administrator.
