Retirement Claim Form: This document is completed by members upon reaching the allowable retirement ages and are registered under the pension fund. Upon completion, the retiree submits this form to the employer who then needs to verify that all requirements have been completed and attachments provided. Once done, this form accompanied by the Claim notification form should be submitted by the Employer to the Fund/Administrator.
