Withdrawal Claim Form: This document is completed by members upon ending/terminating their employment and are registered under the pension fund. Upon completion, the member submits this form to the employer who then needs to verify that all requirements have been completed and attachments provided. Once done, this form accompanied by the Claim notification form should be submitted by the Employer to the Fund/Administrator.
